Student Demographic information
If you have any corrections or changes to your student demographic information (such as a change of address, phone number, or email address), please complete and submit the Student Information Change Form (pdf).

Address changes must include proof of residence (copy of a gas, water or electric bill.) If rent/lease includes all utiities, include a copy of the rental/lease agreement.
