New Student Enrollment
Students enrolled in Kaiser High School must reside within the designated boundaries as set by the Hawai'i State Department of Education.
- The boundaries are from Makapu'u Lighthouse to Moomuku Place at the west end of Kuliouou Valley.
- The parents or legal guardians of students must establish permanent residency within these boundaries.
- Students who are in the process of moving to the Kaiser district will not be allowed to register without a DROA (for home purchase) or adequate proof of residency

Students transferring from a Hawaii Public DOE or Hawaii Public Charter School
Students who were last enrolled in a Hawaii Department of Education or Hawaii Charter school before transferring to Kaiser High School must submit the following:
- Enrollment Form (SIS-10) (fillable pdf) for each child that will attend Kaiser High School
- Proof of residency (Utility bill with parents name and address in Kaiser district)
- Form 211 (Certificate of Release with transcript and clearance form) from withdrawing school.
- Legal Parent/Guardian picture identification
Students transferring from Private School, Out-of-State, or Out-of-County
Students who last attended a private, out-of-state or out-of-country school need to submit the Enrollment Form (SIS-10, fillable pdf) and all other documents listed on the Registration Requirements
Health Record
Hawaii Department of Education, Form 14, is completed by a Physician, APRN, PA or Clinic to include student’s medical status, examination information, immunizations, and tuberculosis evaluation.
Students continuing in the Hawaii DOE system may already have a Form 14 in student’s file. New students to the Hawaii DOE system will need a Form 14 or equivalent.
Initial form must be brought in person to the Kaiser Registrar's office. Registrar's Office house are 7:30am - 3:30pm, Monday - Friday, excluding state & federal holidays.
